Pennsylvania's Nursing Home Transition Project is a program that
serves older adults and persons with physical disabilities who
currently reside in nursing facilities. A Pennsylvania
Department of Welfare (DPW) program, Nursing Home Transition
helps eligible nursing facility residents return home to live
independently in the community. The Northeast Pennsylvania
Center for Independent Living (NEPACIL) functions as a
collaborative partner in Nursing Home Transition with Area
Agencies on Aging (AAA's), with NEPACIL focusing its efforts on
consumers who are under sixty years of age.
NEPACIL is working hand in hand with Area Agencies on Aging in
seven counties in Northeast Pennsylvania, including:
Lackawanna
Luzerne
Wyoming
Wayne
Monroe
Pike
Carbon
Individuals and organizations that NEPACIL works closely with
during the transition process include:
Nursing facility residents
Family and friends of residents
Social workers
Nursing facility administration
Housing specialists and public/private housing agencies
Area Agencies on Aging
Social Service agencies that may provide assistance for a
consumers specific needs
The main goal of Nursing Home Transition is to help nursing
facility residents access the services they require in order to
return home and live independently. Some of the services that
may be available to consumers include:
Attendant care
Affordable housing
Essential furnishings/household setup items
Assistance with security deposits
Medical equipment and supplies
Home modifications
Personal Emergency Response Systems
